Have the space missions carried out to date ended up as well as possible, and, for that reason, there haven’t fortunately been any fatalities in space?

There certainly HAVE been fatalities in space. One took place right over my home when the shuttle Columbia burned up on re-entry, killing all aboard. Another happened when the shuttle Challenger blew up when it launched, killing all aboard. Where have you been?
